Patient requires removal of impacted tooth #38 and retained roots of tooth #35.

Treatment Plan
LAOP removal of impacted tooth #38 and retained roots of tooth #35.
Risks of paresthesia to the lower lip and tongue, as well as OAP/OAF, explained.
Quotation given.
Patient understood and agreed to proceed.

Anesthesia
LA block (L) IDN, LN, and LBN administered.
Surgical Procedure
1. Tooth #38:
Incision and buccal flap raised.
Bone guttering performed.
Tooth sectioned and elevated out in eight pieces.
IDN noted on the lingual surface of the socket; photo taken for patient.
Irrigation with saline.
BSS x 03 placed for primary closure.
Hemostasis achieved, and post-operative instructions (POI) given.
2. Tooth #35:
Incision and buccal + lingual flap raised.
Bone guttering performed.
Root remnants fully removed.
Irrigation with saline.
BSS x 02 placed for primary closure.
Hemostasis achieved, and post-operative instructions (POI) given.
Post-operative Assessment
Post-operative CBCT taken.
IDN plotted and intact.
No tooth remnants noted at the #35 and #38 sites.
Patient reassured.
