Chief Complaint
Patient requests removal of tooth #38 due to multiple episodes of pericoronitis.
Clinical Findings
CBCT reviewed.
Identified risks include curved thin roots lodged in dense lingual bone plate.
Treatment Plan
Explain risks and procedures to the patient.
Patient understood and agreed to proceed.
Informed consent obtained.
Pre-operative Assessment
CBCT reviewed to assess the position and condition of tooth #38 and its roots.
Anesthesia
Local anesthesia: inferior dental nerve block, lingual nerve, and long buccal nerve block.
Surgical Procedure
Patient cleaned and draped.
Incision made, buccal flap raised.
Bone guttering performed.
Tooth #38 sectioned and elevated out in two pieces.
Two root tips fractured and retained deep in the socket.
Distal root tip picked out.
Mesial root tip initially not visible.
CBCT taken to confirm the position of the mesial root tip.
Bone guttering to gain access to the mesial root tip.
Mesial root tip picked out.
Curettage and irrigation with saline.
Bone substitute placed for primary closure.
Hemostasis achieved and post-operative instructions given.
