Chief Complaint

Patient requests removal of tooth #38 due to multiple episodes of pericoronitis.


Clinical Findings

  • CBCT reviewed.

  • Identified risks include curved thin roots lodged in dense lingual bone plate.

Treatment Plan

  • Explain risks and procedures to the patient.

  • Patient understood and agreed to proceed.

  • Informed consent obtained.

Pre-operative Assessment

  • CBCT reviewed to assess the position and condition of tooth #38 and its roots.


Anesthesia

  • Local anesthesia: inferior dental nerve block, lingual nerve, and long buccal nerve block.


Surgical Procedure

  • Patient cleaned and draped.

  • Incision made, buccal flap raised.

  • Bone guttering performed.

  • Tooth #38 sectioned and elevated out in two pieces.

  • Two root tips fractured and retained deep in the socket.

  • Distal root tip picked out.

  • Mesial root tip initially not visible.

  • CBCT taken to confirm the position of the mesial root tip.

  • Bone guttering to gain access to the mesial root tip.

  • Mesial root tip picked out.

  • Curettage and irrigation with saline.

  • Bone substitute placed for primary closure.

  • Hemostasis achieved and post-operative instructions given.
