Chief Complaint

Pain and swelling associated with impacted tooth #48.

Clinical Findings

  • Tooth #48 was completely buried in bone and impacted.

  • The patient complained of pain and swelling and requested the removal of tooth #48.

Treatment Plan

Surgical removal of the impacted tooth #48.

Pre-operative Assessment

  • A pre-operative CBCT scan was taken to assess the position and condition of the impacted tooth.

  • The patient was informed of the procedure, including the potential risks of paresthesia to the lower lip and tongue. The patient understood and agreed to proceed with the surgery.

  • Informed consent was obtained.

Anesthesia

Local anesthesia blocks were administered to the right inferior dental nerve (IDN), lingual nerve (LN), and long buccal nerve (LBN).

Surgical Procedure

  1. The patient was cleaned and draped.

  2. An incision was made, and a buccal flap was raised.

  3. Bone guttering was performed to access the impacted tooth.

  4. The tooth was sectioned and elevated out in two pieces.

  5. The socket was curetted and irrigated with saline.

  6. Primary closure was achieved with 04 BSS sutures.

  7. Hemostasis was confirmed, and post-operative instructions (POI) were provided.
