Surgical Removal
Extraction, Buried and Impacted

Chief Complaint
Patient complains of pain and swelling associated with tooth #18.
Clinical Findings
Tooth #18 is 90% buried in bone and impacted.
Pain and swelling in the area of tooth #18.
Treatment Plan
Surgical removal of tooth #18.
Extraction of adjacent tooth #17.
Pre-operative Assessment
Pre-operative CBCT taken for evaluation.
Patient informed about the risks, including potential oral-antral perforation (OAP) or fistula (OAF).
Procedures were explained, and the patient consented to proceed.
Anesthesia
Local anesthesia administered to teeth #17 and #18.
Surgical Procedure
Extraction of Tooth #17
The patient was prepared with cleaning and draping.
An incision was made and the buccal flap raised.
Tooth #17 was extracted using elevators.
2. Extraction of Impacted Tooth #18
Additional bone guttering was performed to access tooth #18.
The tooth was sectioned and removed in two pieces.
Curettage and saline irrigation were done.
Seven BSS sutures were placed for primary closure to achieve hemostasis.
3. Post-Operative Care
Post-operative CBCT confirmed no tooth remnants and that the maxillary sinus was intact and not involved in the surgery.
The patient was reassured and provided with post-operative instructions.
