Wisdom Tooth Removal for #18

Chief Complaint

Patient complains of pain and swelling associated with tooth #18.

Clinical Findings

  • Tooth #18 is 90% buried in bone and impacted.

  • Pain and swelling in the area of tooth #18.

Treatment Plan

  • Surgical removal of tooth #18.

  • Extraction of adjacent tooth #17.

Pre-operative Assessment

  • Pre-operative CBCT taken for evaluation.

  • Patient informed about the risks, including potential oral-antral perforation (OAP) or fistula (OAF).

  • Procedures were explained, and the patient consented to proceed.

Anesthesia

Local anesthesia administered to teeth #17 and #18.

Surgical Procedure

  1. Extraction of Tooth #17

  • The patient was prepared with cleaning and draping.

  • An incision was made and the buccal flap raised.

  • Tooth #17 was extracted using elevators.

2. Extraction of Impacted Tooth #18

  • Additional bone guttering was performed to access tooth #18.

  • The tooth was sectioned and removed in two pieces.

  • Curettage and saline irrigation were done.

  • Seven BSS sutures were placed for primary closure to achieve hemostasis.

3. Post-Operative Care

  • Post-operative CBCT confirmed no tooth remnants and that the maxillary sinus was intact and not involved in the surgery.

  • The patient was reassured and provided with post-operative instructions.
