Wisdom Tooth Extraction for Tooth #38

Surgical Removal

Chief Complaint

Patient presented with mother for removal of tooth #38.

Clinical Findings

  • Reviewed CBCT with patient's mother.

  • Inferior dental nerve (IDN) plotted and shown in relation to the apices of tooth #38; lingual of apices.

  • Buccal abscess noted on teeth #38 and #37.

  • Mild facial swelling on the left-hand side.

Treatment Plan

Extraction of tooth #38.

Pre-operative Assessment

  • Risks and procedures for local anesthesia (LA) operation for removal of tooth #38 explained.

  • Patient and patient's mother agreed to proceed.

  • Informed consent for LA operation on tooth #38 obtained.

Anesthesia

Local anesthesia block: left inferior dental nerve (IDN), lingual nerve (LN), and long buccal nerve (LBN).

Surgical Procedure

  1. Patient cleaned and draped.

  2. Incision made.

  3. Pus drained.

  4. Irrigation with saline.

  5. Buccal flap raised.

  6. Bone guttering performed.

  7. Tooth #38 sectioned and elevated out in three pieces.

  8. Curettage performed.

  9. Granulation tissues removed.

  10. Irrigation with saline.

  11. BSS x 02 placed for primary closure.

  12. Hemostasis achieved and post-operative instructions provided.

Post-operative Assessment

  • Post-operative CBCT taken.

  • IDN spared, no tooth remnants noted.
