Risks of curved, thin roots lodged in dense lingual bone plate potentially breaking were explained.
Procedures explained.
Patient understood and agreed to proceed.
Informed consent obtained.
Treatment Plan
Extraction of wisdom tooth #38.
Pre-operative Assessment
Body Temperature: 36.4°C.
Appointment scheduled for 10:45 am; patient arrived at 10:34 am.
Anesthesia
Local anesthesia administered: Left Inferior Dental Nerve (IDN), Lingual Nerve (LN), and Long Buccal Nerve (LBN) blocks.
Surgical Procedure
Patient cleaned and draped.
Incision made and buccal flap raised.
Bone guttering performed.
Tooth sectioned.
Tooth #38 elevated out in 2 pieces.
Two root tips fractured and retained deep in the socket.
Distal root tip picked out.
Mesial root tip not visible; CBCT taken to confirm position.
Bone guttering performed to gain access to mesial root tip.
Mesial root tip picked out.
Socket curetted and irrigated with saline.
BSS x 03 placed for primary closure.
