Chief Complaint

Patient complains of pain and swelling in the area of tooth #38.

Clinical Findings

Tooth #38 is completely buried in bone and impacted.

Treatment Plan

  • Removal of tooth #38 under local anesthesia (LA)

  • Extraction of tooth #28 under local anesthesia (LA XAP)

Pre-operative Assessment

  • Pre-operative CBCT taken

  • Patient advised of the procedures and associated risks, including paresthesia to the lower lip and tongue, and potential for oroantral perforation/fistula (OAP/OAF)

  • Procedures explained, and informed consent obtained

Anesthesia

  • LA block: left inferior dental nerve (IDN), lingual nerve (LN), and long buccal nerve (LBN)

  • LA infiltration for tooth #28

Surgical Procedure

  • Patient cleaned and draped

  • Incision made, buccal flap raised

  • Bone guttering performed

  • Tooth #38 sectioned and elevated out in three pieces

  • Currettage and irrigation with saline

  • Bioactive synthetic scaffold (BSS) placed x5 for primary closure

  • Hemostasis achieved, post-operative instructions given

  • LA extraction of tooth #28

  • Irrigation with saline

  • Hemostasis achieved, post-operative instructions given
