Surgical Removal

Chief Complaint

Pain and swelling due to retained infected carious roots of tooth #36.

Clinical Findings

Tooth #36 has very tender-to-pressure (TTP) roots, with swelling around the retained roots. Mobility and probing depth are within normal limits. Caries extend to the pulp and are gross and extensive, reaching the crestal level. Sound bone level observed.

Treatment Plan

Local anesthesia (LA) removal of retained roots of tooth #36.

Pre-operative Assessment

  • Pre-op CBCT taken.

  • Risks of paraesthesia to the lower lip and tongue discussed.

  • Procedures explained.

  • Patient understood and agreed to proceed.

  • Informed consent obtained.

Anesthesia

Local anesthesia administered.

Surgical Procedure

  1. Patient cleaned and draped.

  2. Incision made, buccal flap raised.

  3. Bone guttering performed.

  4. Tooth sectioned and #36 elevated out in two pieces.

  5. Curettage performed, followed by irrigation with saline.

  6. BSS x 06 placed for secondary closure.

  7. Haemostasis achieved.

Post-operative Assessment

  • Post-op CBCT taken, showing no remnants. Inferior dental nerve (IDN) and adjacent teeth not involved.

  • Patient reassured.
