Pre-operative CBCT taken; teeth #14, #15, #36, #34, #38, #44, and #45 deemed hopeless.
Treatment Plan
Local anesthesia (LA) and surgical removal of retained roots in #14, #15, and #36.
Local anesthesia (LA) and examination (XAP) of periodontally involved teeth #34, #38, #44, and #45.
Pre-operative Assessment
Risks of paresthesia to lower lip and tongue discussed with the patient.
Procedures explained thoroughly.
Patient understood and agreed to proceed.
Informed consent obtained.
Pre-operative CBCT reviewed.
Anesthesia
Local anesthesia block administered to left and right inferior dental nerve (IDN), lingual nerve (LN), and long buccal nerve (LBN).
Local infiltration for #14, #15.
Surgical Procedure
1. Patient cleaned and draped.
2. Incision made and buccal flap raised.
3. Bone guttering performed.
4. Tooth sectioning and removal of retained roots from #14, #15, and #36, elevated in four pieces.
5. Curettage and irrigation with saline.
6. Bone grafting material (BSS) applied:
- BSS x 04 for #15 and #14 sites.
- BSS x 02 for #36 site.
7. Haemostasis achieved, and post-operative instructions provided.
8. Local anesthesia XAP performed for #34, #38, #44, and #45.
9. Irrigation with saline and haemostasis achieved.
