Chief Complaint
#14: Gross caries extending to the pulp. Severe tenderness to percussion, slight metallic sound on percussion. Retained root(s) noted.
#25: Periodontal and endodontic involvement. Severe tenderness to percussion, mobility II, probing depth to apex. Tooth deemed hopeless.
Clinical Findings
#14: Very tender to percussion (TTP), slight metallic sound on percussion. Mobility and probing depth within normal limits. Dense bone and possible ankylosis noted.
#25: Very tender to percussion (TTP), mobility II, probing depth to apex. Tooth is considered hopeless

Local anesthesia (LA) and surgical removal of retained root(s) of #14
Extraction of #25
Pre-operative Assessment
Pre-operative CBCT performed.
Risks of paresthesia to lower lip and tongue explained.
Procedures explained to the patient.
Patient understood and agreed to proceed.
Informed consent obtained.
Anesthesia
Local anesthesia administered for teeth #14 and #25.
Surgical Procedure
1. X-ray of #25 taken.
2. Pus drained from #25.
3. Socket irrigated with saline.
4. Bone grafting material (BSS x 03) placed.
5. Haemostasis achieved.
6. Incision and buccal flap raised for #14.
7. Bone guttering performed.
8. Tooth sectioned and elevated in two pieces.
9. Curettage and irrigation with saline.
10. Bone grafting material (BSS x 03) placed for closure.
11. Haemostasis and post-operative instructions provided.
