Surgical Removal

Chief Complaint

Patient complains of a dislodged crown on tooth #16.

Clinical Findings

  • #16 fractured with recurrent caries.

  • Retained roots with gross decay.

  • Roots are long, thin, and divergent.

  • #16 firm retained roots.

  • Pre-operative CBCT confirms:

    • 3-rooted #16 with extensive caries.

  • Additional findings:

    • #46: prior crowning with distal cervical radiolucency—KIV recurrent caries/leakage.

    • #17 (MO): AR with possible leakage—KIV crowning.

Treatment Plan

  • LA OP surgical removal of retained roots of #16.

  • Risks, alternatives, and expected outcomes explained.

  • Risk of oro-antral penetration and fistula discussed.

  • Patient understood and agreed to proceed.

Pre-operative Assessment

  • Pre-operative CBCT taken.

  • Adjacent structures assessed—maxillary sinus evaluated for proximity.

  • Informed consent obtained.

Anesthesia

LA block: Right IDN, LN & LBN.

Surgical Procedure

  1. Patient cleaned and draped.

  2. Incision made; buccal flap raised.

  3. Bone guttering performed—bone noted to be very dense.

  4. Tooth sectioned.

  5. #16 removed in five pieces using elevation technique.

  6. Curettage of socket; saline irrigation.

  7. Closure with 4/0 Vicryl × 5 sutures for secondary closure.

  8. Hemostasis achieved; POI given.

  9. Post-op CBCT taken.

Post-operative Assessment

  • No retained fragments noted.

  • Maxillary sinus not involved.

  • Adjacent teeth unaffected.

Patient satisfied with treatment outcome.
