Long, wide, flattened mesiobuccal root
Thin and curved distolingual root

Surgical removal of retained #17 roots under local anesthesia

Pre-operative Assessment
CBCT reviewed prior to surgery.
Root morphology and relationship to adjacent anatomical structures assessed.
Risks of oroantral perforation, bleeding, swelling, infection, retained fragments, and post-operative discomfort discussed.
Px understood and agreed to proceed.
Informed consent obtained.
Anesthesia
Local anesthesia administered to surgical site.
Surgical Procedure
Px cleaned and draped in sterile manner.
Incision made and buccal flap raised.
Bone guttering performed.
Tooth sectioning carried out to facilitate removal.
#17 retained roots elevated and removed in 5 pieces.
Socket curettage performed and irrigated thoroughly with saline.
BSS x2 placed for secondary closure.
Haemostasis achieved.
POI given.
Post-operative Assessment
Post-operative CBCT taken.
No retained root remnants noted.
Maxillary sinus and adjacent teeth not involved during surgery.
Healing satisfactory post-operatively.
Px reassured and discharged in stable condition.
