Chief Complaint
Patient presents with pain and swelling, requesting removal of impacted tooth #38.
Clinical Findings
Tooth #38: Impacted.
Tooth #37: Perio-endodontically involved, with difficulty in extraction due to blockage from the impacted #38.
Pain and swelling noted in the affected area.
Treatment Plan
Local anesthesia (LA) and surgical removal of impacted and non-functional tooth #38.
Local anesthesia (LA) and X-ray examination (XAP) of tooth #37.
Pre-operative Assessment
Pre-operative CBCT reviewed.
Risks of paresthesia to the lower lip and tongue discussed.
Procedures explained in detail.
Patient agreed to the proposed treatment plan.
Informed consent obtained.
Anesthesia
Local anesthesia block administered to left inferior dental nerve (IDN), lingual nerve (LN), and long buccal nerve (LBN).
Surgical Procedure
1. Patient cleaned and draped.
2. Incision made and buccal flap raised.
3. X-ray examination (XAP) of tooth #37 performed; path of withdrawal blocked by impacted tooth #38.
4. Tooth #37 luxated but remained stuck.
5. Bone guttering performed.
6. Tooth #38 sectioned and elevated in two pieces.
7. Tooth #37 elevated in one piece.
8. Curettage performed and gross granulation tissue removed.
9. Irrigation with saline.
10. Bone grafting material (BSS x 08) applied for primary closure.
11. Haemostasis achieved, and post-operative instructions provided.
