Surgical Removal

Chief Complaint

Patient reports a cavity on the upper right last tooth and food trap on the lower right last tooth. 

The impacted tooth #48 is causing severe food trap and pain.

Clinical Findings

Tooth #48: Completely buried in bone, carious cavity, and causing food trap.

Tooth #18: Wisdom tooth requiring extraction.

Patient reports pain and swelling.

Treatment Plan

LA OP removal of impacted tooth #48.

LA XAP of tooth #18.

Pre-operative Assessment

Pre-operative CBCT taken to assess tooth #48 and #18.

Risks of parasthesia to the lower lip and tongue, as well as OAP/OAF, explained.

Procedures and costs explained.

Informed consent obtained.

Anesthesia

LA block administered to left IDN, LN, and LBN.

LA infiltration for tooth #18.

Surgical Procedure

  1. Patient cleaned and draped.

  2. Incision made and buccal flap raised.

  3. Bone guttering performed.

  4. Tooth sectioning executed.

  5. Tooth #48 elevated in 9 pieces.

  6. Curettage and saline irrigation completed.

  7. 5/0 BSS x 07 placed for primary closure.

  8. Haemostasis achieved.

Post-operative Assessment

Post-operative CBCT taken.

No tooth remnants noted.

IDN and max sinus intact and not involved in the surgery.

Patient reassured.
