Px c/o pain and discomfort associated with #38 region.

Clinical Findings

  1. CBCT reviewed.
    Left IDN plotted.

#38

  • Buried and vertically impacted

  • Tooth presence detected on probing

  • Not visible intra-orally

  • Swelling noted distal to #37

Treatment Plan

  • Surgical removal of buried and impacted #38 under local anesthesia

Risks, benefits, procedural steps, and post-operative expectations discussed with Px.

Pre-operative Assessment

  1. CBCT assessment performed prior to surgery.
    Relationship of #38 to left IDN evaluated and plotted.

Risks of paraesthesia to lower lip and tongue, OAP/OAF, swelling, pain, infection, and bleeding explained.
Px understood and agreed to proceed.
Informed consent obtained.

Anesthesia

LA block administered to left IDN, LN, and LBN.

Surgical Procedure

  1. Px cleaned and draped in sterile manner.

Incision made and buccal flap raised.
Bone guttering performed to expose impacted tooth.
Tooth sectioning carried out.

#38 elevated and removed in 6 pieces.

Socket curettage performed and irrigated thoroughly with saline.
BSS x3 placed for primary closure.
Haemostasis achieved.
POI given.

Post-operative Assessment

Post-operative CBCT taken.
No residual tooth remnants noted.
IDN intact and not involved during surgery.
Healing satisfactory post-operatively.
Px reassured and discharged in stable condition.
