Chief Complaint
Patient has a buried and inverted tooth #48, impacted with #47, and an odontogenic cyst around the coronal of #48.
Clinical Findings
Tooth #48 is completely buried in bone, inverted, and impacted with tooth #47.
An odontogenic cyst is present around the coronal of #48, distalizing #48 into the ascending ramus.
Treatment Plan
Removal of buried, inverted, and impacted tooth #48.
Enucleation and removal of the odontogenic cyst around the coronal of #48.
Bone grafting to improve bone regeneration distal to #47.
Pre-operative Assessment
Pre-operative CBCT reviewed.
Risks of paresthesia to the lower lip and tongue were explained, and the procedure was discussed.
Patient agreed to proceed with the treatment.
Informed consent obtained.
Anesthesia
Local anesthesia administered: right inferior dental nerve (IDN), lingual nerve (LN), and long buccal nerve (LBN) block.
Surgical Procedure
Patient cleaned and draped.
Incision made and buccal flap raised.
Bone guttering performed.
Tooth #48 sectioned and elevated in 11 pieces.
Curettage performed; odontogenic cyst development sac around #48 fully enucleated and removed.
Irrigation with saline conducted.
Bone grafting with EthOss 1.0cc artificial bone grafting material placed to address bony defect.
Flaps released and seven BSS (Bone Spongiosa Substitute) placed for primary closure.
Haemostasis achieved, and post-operative instructions given.
