Chief Complaint
Patient reports pain and swelling. Tooth #47 shows caries potentially extending to the pulp. Tooth #48 is impacted, and tooth #18 is non-functional with poor oral hygiene.
Clinical Findings
Tooth #47: Very tender to palpation, caries extending to the pulp, mobility and probing depth within normal limits.
Tooth #48: Slight tenderness to palpation, mobility and probing depth within normal limits.
Tooth #18: Non-functional with poor oral hygiene.
Treatment Plan
Root canal treatment (RCT) for tooth #47.
Removal of impacted tooth #48.
Extraction of non-functional tooth #18.
Pre-operative Assessment
Pre-operative CBCT taken.
IDN plotted.
Risks and procedures explained to the patient.
Patient agreed to proceed with the recommended treatments.
Anesthesia
Local anesthesia block for right IDN, LN, and LBN.
Local anesthesia infiltration for tooth #18.
Surgical Procedure
Patient cleaned and draped.
Incision made and buccal flap raised.
Bone guttering performed.
Tooth #48 sectioned and elevated in 2 pieces.
Tooth #47 elevated in total.
Curettage and saline irrigation performed.
BSS x 06 placed for primary closure.
Haemostasis achieved and post-operative instructions provided.
