Chief Complaint
Patient complains of pain and swelling in the area of tooth #48. The patient requested the removal of the impacted tooth.
Clinical Findings
Tooth #48 deeply and horizontally impacted, not visible intraorally.
Pain and swelling associated with the impacted tooth.
Treatment Plan
Removal of impacted tooth #48 under local anesthesia.
Extractions of teeth #18 and #17 under local anesthesia.
Pre-operative Assessment
Pre-operative CBCT taken and reviewed.
Patient informed of the risks of paresthesia to the lower lip and tongue, as well as the possibility of oroantral perforation (OAP) or oroantral fistula (OAF).
The procedure was explained, and the patient understood and agreed to proceed.
Informed consent obtained.
Anesthesia
Local anesthesia administered: right inferior dental nerve (IDN), lingual nerve (LN), and long buccal nerve (LBN) block.
Local anesthesia infiltration for teeth #18 and #17.
Surgical Procedure
Patient cleaned and draped.
Incision made, buccal flap raised.
Bone guttering performed.
Tooth #48 sectioned and elevated in 8 pieces.
Curettage and irrigation with saline.
Five BSS (Bone Spongiosa Substitute) placed for primary closure.
Haemostasis achieved, and post-operative instructions given.
Extractions of teeth #18 and #17 completed successfully under local anesthesia.
Irrigation with saline and haemostasis achieved.
