Chief Complaint
Patient presents with pain and swelling in the region of #48.
Patient requests removal of #48.
Tooth #18 is non-functional with poor oral hygiene and large caries.
Clinical Findings
Tooth #48: Impacted with deep carious lesion.
Tooth #18: Non-functional with extensive caries.
Pre-operative CBCT reviewed.
Treatment Plan
Surgical removal of tooth #48.
X-ray and evaluation of tooth #18.
Pre-operative Assessment
Risks and procedures explained.
Informed consent obtained.
Patient agreed to proceed with the treatment.
Anesthesia
Local anesthesia administered via block to left IDN, LN, and LBN.
Local anesthesia infiltration for #18.
Surgical Procedure
1. Patient cleaned and draped.
2. Incision made, and buccal flap raised.
3. Bone guttering performed.
4. Tooth sectioning:
- #48: Elevated in seven pieces.
5. Curettage and irrigation with saline.
6. BSS x 04 placed for primary closure.
7. Haemostasis achieved and post-operative instructions provided.
8. XAP for #18:
- Irrigation with saline.
- Haemostasis and post-operative instructions provided.

Post-operative Assessment
Post-operative CBCT taken.
No tooth remnants noted.
IDN and maxillary sinus intact and not involved in surgery.
Patient reassured.
