Impacted Wisdom Tooth, Extraction

Chief Complaint
Patient reported pain in the lower right posterior region.
Clinical Findings
Tooth #48 is impacted, causing discomfort and pushing against the adjacent tooth.
Tooth #47 is carious with a distal-occlusal cavity and has supra-erupted due to the impact from tooth #48.
Treatment Plan
Removal of both teeth #47 and #48 as per patient’s request.
Pre-operative Assessment
A pre-operative CBCT scan was performed to assess the impacted tooth and the condition of the adjacent structures.
The patient was informed about the potential risks, including paresthesia to the lower lip and tongue. The procedures and treatment options were explained in detail.
The patient understood the risks and consented to the removal of both teeth. Informed consent was obtained.
Anesthesia
Local anesthesia blocks were administered for the right inferior dental nerve (IDN), lingual nerve (LN), and long buccal nerve (LBN).
Surgical Procedure
The patient was prepped and draped in a sterile manner.
An incision was made, and a buccal flap was raised to expose the surgical site.
Bone guttering was conducted to gain access to the impacted tooth.
Tooth #48 was sectioned and extracted in two pieces.
The area was curetted and thoroughly irrigated with saline.
Tooth #47 was then extracted using local anesthesia.
The site was irrigated, and primary closure was achieved with 05 BSS sutures.
Hemostasis was ensured, and post-operative instructions were provided.
