Surgical Removal

Chief Complaint

Prior episodes of pain and swelling due to impacted tooth #38.

Clinical Findings

Tooth #38 is completely buried in bone and impacted.

Treatment Plan

Local anesthesia (LA) removal of tooth #38 with bone grafting.

Pre-operative Assessment

  • Pre-op CBCT reviewed.

  • Risks of paraesthesia to the lower lip and tongue discussed.

  • Procedures explained.

  • Patient understood and agreed to proceed.

  • Informed consent obtained.

Anesthesia

Local anesthesia administered with left inferior dental nerve (IDN) block, lingual nerve (LN) block, and long buccal nerve (LBN) block.

Surgical Procedure

  1. Patient cleaned and draped.

  2. Incision made, buccal flap raised.

  3. Bone guttering performed.

  4. Tooth sectioned and #38 elevated out in eight pieces.

  5. Curettage performed, followed by irrigation with saline.

  6. BSS x 04 placed for primary closure.

  7. Haemostasis achieved.

Post-operative Assessment

  • Post-op CBCT taken, showing dense bone graft with no tooth remnants.

  • Patient reassured.
