Chief Complaint

Removal of deeply impacted and buried tooth #28.

Clinical Findings

  • Tooth #28 deeply impacted, approximately 90% buried in bone.

  • Pre-operative CBCT reviewed.

Treatment Plan

  • Extraction of tooth #28.

  • Bone grafting with 1.0 cc EthOss artificial bone grafting material for the extraction site and distal area of #27.

Pre-operative Assessment

  • Quotation provided.

  • Risks of oral antral perforation (OAP) and oral antral fistula (OAF) discussed.

  • Procedures explained; patient consent obtained.

Anesthesia

  • Local Anesthesia (LA) block administered to left inferior dental nerve (IDN), lingual nerve (LN), and long buccal nerve (LBN).

  • LA infiltration at site #28.

Surgical Procedure

  1. Patient cleaned and draped.

  2. Incision made; buccal flap raised.

  3. Bone guttering performed.

  4. Tooth sectioned and elevated in 07 pieces.

  5. Curettage performed; granulation tissues removed and site irrigated with saline.

  6. Bone grafting with EthOss material applied to the site and distal area of #27.

  7. Primary closure achieved with BSS x 05.

  8. Hemostasis ensured.
