Tooth #27: Fractured with retained roots
Pre-operative CBCT shows retained roots with a large periapical lesion
Additional findings include a vertically impacted tooth #48 with a cystic lesion
Treatment Plan
Advised extraction of tooth #27 under local anesthesia
Patient agreed to the procedure

Anesthesia
Local anesthesia infiltration around tooth #27
Surgical Procedure
Patient prepared with standard cleaning and draping
Buccal flap raised with an incision
Bone guttering performed
Tooth #27 sectioned and removed in three pieces
Site curetted and irrigated with saline
Secondary closure with BSS x 05
Hemostasis achieved
