Chief Complaint

Patient reports pain and swelling in the area of tooth #27, associated with retained roots.

Clinical Findings

  • Tooth #27: Fractured with retained roots

  • Pre-operative CBCT shows retained roots with a large periapical lesion

  • Additional findings include a vertically impacted tooth #48 with a cystic lesion

Treatment Plan

  • Advised extraction of tooth #27 under local anesthesia

  • Patient agreed to the procedure

Pre-operative Assessment

  • CBCT confirmed the presence of retained roots and a large periapical lesion on tooth #27

  • Patient informed of potential risks, including oroantral perforation and oroantral fistula

  • Informed consent obtained

Anesthesia

Local anesthesia infiltration around tooth #27

Surgical Procedure

  1. Patient prepared with standard cleaning and draping

  2. Buccal flap raised with an incision

  3. Bone guttering performed

  4. Tooth #27 sectioned and removed in three pieces

  5. Site curetted and irrigated with saline

  6. Secondary closure with BSS x 05

  7. Hemostasis achieved
