Chief Complaint
Patient complained of pain and swelling associated with impacted tooth #48.
Clinical Findings
Tooth #48: Completely buried in bone, impacted.
Patient requested removal of tooth #48.
Treatment Plan
Surgical removal of impacted tooth #48.
Pre-operative Assessment
Pre-operative CBCT taken to assess the position of tooth #48.
Risks of paresthesia to lower lip and tongue discussed.
Procedures explained to the patient.
Patient understood and agreed to proceed.
Informed consent obtained.
Anesthesia
Local anesthesia block administered: Right Inferior Dental Nerve (IDN), Lingual Nerve (LN), and Long Buccal Nerve (LBN).
Surgical Procedure
Patient cleaned and draped.
Incision made and buccal flap raised.
Bone guttering performed.
Tooth sectioned to facilitate removal.
Tooth #48 elevated out in two pieces.
Socket curettage and irrigation with saline performed.
BSS x 04 placed for primary closure.
Achieved hemostasis and provided post-operative instructions (POI).

Post-operative Assessment
Post-operative CBCT taken to confirm complete removal of tooth remnants.
Inferior Dental Nerve (IDN) intact and not involved in the surgery.
Patient reassured.
