Chief Complaint

Pain in the lower right posterior jaw region.

Clinical Findings

  • Partially erupted #47

  • Large, thick operculum obstructing proper eruption

  • Pericoronitis with swelling and pus exudate

  • Localised soft tissue inflammation

  • Pain reproduced upon examination

  • CBCT confirmed partial eruption and operculum coverage.

Treatment Plan

Surgical exposure of tooth #47 with excision of operculum and soft tissue recontouring.

Pre-operative Assessment

  • Pre-op photographs taken

  • Risks, benefits, and procedure explained

  • Quotation provided

  • Patient and mother understood and agreed

  • Informed consent obtained for surgical exposure of #47

Anesthesia

Local anesthesia infiltration around #47.

Surgical Procedure

  • Patient cleaned and draped

  • Incision made

  • Thick operculum over #47 excised

  • 980 nm diode laser used for soft tissue recontouring around #48 and to achieve hemostasis

  • Vitamin E applied to promote soft tissue healing

  • Post-operative instructions given

Post-operative Assessment

  • Post-op photographs taken

  • Good exposure of #47 achieved

  • Hemostasis stable

  • Patient discharged in stable condition with POI
