Revision Tongue-Tie Release (Lower Lingual Frenectomy)

Surgical Procedure

Chief Complaint

  • Reduced tongue extension due to soft tissue adhesion after previous lingual frenectomy; patient desires restoration of prior post-operative mobility.

Clinical Findings

  • Scarring and soft tissue adhesion at lower lingual frenum

  • Decreased tongue protrusion compared to immediate post-operative result

  • Restriction affecting functional tongue movement

Treatment Plan

  • Revision lingual frenectomy using diode laser, with suturing to minimise re-adhesion.

Pre-operative Assessment

  • Risks, procedure steps, and expected outcome explained

  • Patient understood and agreed to proceed

  • Informed consent obtained for lower lingual frenectomy revision

Anesthesia

Local anesthesia via infiltration.

Surgical Procedure

  • Patient cleaned and draped

  • Lingual frenum placed under tension

  • Release performed using 15C blade from base of mouth toward tongue tip

  • 980 nm diode laser applied for finishing and hemostasis

  • BSS × 7 sutures placed to close incision and reduce risk of tissue adhesion

  • Post-operative instructions provided

Post-operative Assessment

  • Adequate release achieved

  • Hemostasis stable

  • Good tongue mobility immediately post-op

  • Healing trajectory discussed with emphasis on preventing re-adhesion
