Chief Complaint

Patient complains of pain and a broken tooth #32.

Clinical Findings

  • Tooth #48: deep caries to pulp; tender to percussion (TTP).

  • Tooth #43: caries to pulp; TTP.

  • Tooth #31: deep caries to pulp; TTP.

  • Tooth #33: deep caries to pulp; TTP.

  • Retained roots: #32, #35, #36, #44.

Treatment Plan

  • Extraction of teeth #31, #33, #43, #48.

  • Local anesthesia (LA) removal of retained roots #32, #35, #36, #44.

Pre-operative Assessment

  • Pre-operative CBCT taken.

  • Risks including paraesthesia to lower lip and tongue, and oroantral perforation/oral-antral fistula (OAP/OAF) explained to the patient.

  • Procedures and costs explained.

  • Patient understood and agreed to proceed.

  • Informed consent obtained.

Anesthesia

Local anesthesia block (left and right) inferior dental nerve (IDN), lingual nerve (LN), and long buccal nerve (LBN).

Surgical Procedure

  • Patient cleaned and draped.

  • Incision made, and buccal flap raised.

  • Bone guttering performed.

  • Teeth #31, #33, #43, #48 elevated out in total.

  • Retained roots #32, #35, #36, #44 elevated out in pieces.

  • Curettage performed and irrigated with saline.

  • Bone substitute sponge (BSS) placed:

    • #48: 3 sponges.

    • #43, #44: 5 sponges each.

    • #31, #32, #33: 8 sponges each.

    • #35, #36: 8 sponges each.

  • Hemostasis achieved.
