Chief Complaint
Patient complains of pain in the second quadrant.
Clinical Findings
Tooth #25: Retained grossly carious roots, carious to the crestal level.
Very tender to palpation (TTP) and tender to percussion (TTPp).
Mobility and probing depth (PD) within normal limits (WNL).
Treatment Plan
Advised local anesthesia operative procedure (LA OP) for removal of tooth #25:
Risks and procedures explained.
Patient agreed to proceed.
Pre-operative Assessment
Pre-operative CBCT taken and explained to the patient.
Emphasized the risks of sinus perforation.
Patient understood the risks and procedures and agreed to proceed with the treatment today.
Informed consent obtained.
Anesthesia
Local anesthesia infiltration at the #25 site.
Surgical Procedure
Patient cleaned and draped.
Incision made, buccal flap raised.
Bone guttering performed.
Tooth sectioned.
Retained roots of tooth #25 elevated out in two pieces.
Curettage performed, irrigated with saline.
Bioactive synthetic scaffold (BSS) placed x3 for primary closure.
Hemostasis achieved, post-operative instructions given.
