Chief Complaint
Patient presents with non-functional teeth #14, #17, and #18.
Clinical Findings
Tooth #18: Periodontal-endodontic involvement, stable but buccal probing depth to apices.
Tooth #17: Distobuccal caries to pulp at cervical level, non-functional.
Tooth #14: Palatal probing depth to apex, very tender to percussion (vTTP), Mobility I ++.
Treatment Plan
Advised extraction of teeth #14, #17, and #18.
Warned of risks of tooth fracture due to caries and the potential need for local anesthesia (LA) operation for removal of retained root if inaccessible.
Patient agreeable and understands the procedure.
Pre-operative Assessment
Pre-operative CBCT taken.
Surgery planned and discussed with patient.
Informed consent obtained for LA operation for removal of retained root in tooth #17 if necessary.
Anesthesia
Local anesthesia extraction (XAP) for teeth #14, #17, and #18.
Specific consent obtained for LA operation removal of #17 retained root.
Surgical Procedure
Patient cleaned and draped.
Incision made, buccal flap raised.
Bone guttering performed.
Tooth sectioning where necessary.
Extraction of teeth #14, #17, and #18.
Noted #17 fractured and retained distobuccal (db) root inaccessible due to limited mouth opening.
Proceeded with LA operation removal of retained #17 (db) root.
Incision and buccal flap raised.
Bone guttering and tooth sectioning performed.
#17 elevated out completely.
Curettage and irrigation with saline.
Bioactive synthetic scaffold (BSS) placed x8 for secondary closure.
Hemostasis achieved, post-operative instructions given.
