Retained Root Removal for #27
Surgical Removal
Chief Complaint
Pain and swelling associated with retained roots of fractured tooth #27.
Clinical Findings
Tooth #27 had fractured, leaving retained roots, and the patient reported pain and swelling.
The patient requested the removal of tooth #27.
Treatment Plan
Surgical removal of the retained roots of tooth #27.
Pre-operative Assessment
A pre-operative CBCT scan was taken to evaluate the condition of the retained roots and surrounding structures.
The patient was informed of the potential risks, including paresthesia to the lower lip and tongue, and the possibility of oroantral perforation (OAP) or oroantral fistula (OAF). The procedures were thoroughly explained.
The patient understood the risks and procedures and agreed to proceed with the treatment. Informed consent was obtained.
Anesthesia
Local anesthesia infiltration was administered to the area around tooth #27.
Surgical Procedure
The patient was cleaned and draped.
An incision was made, and a buccal flap was raised.
Bone guttering was performed to access the retained roots.
The tooth was sectioned and elevated out in three pieces.
The socket was curetted and irrigated with saline.
Secondary closure was achieved with 03 BSS sutures.
Hemostasis was confirmed, and post-operative instructions (POI) were provided.
Post-operative Assessment
A post-operative CBCT scan confirmed that there were no remnants of the tooth and that the maxillary sinus was not involved.
Additional notes indicated that teeth #16 and #17 might have periodontal-endodontic involvement with a poor periodontal prognosis, possibly requiring extraction (KIV XAP).
