Retained Root Removal for #17

Chief Complaint

Cracked tooth #17 with prior crowning and insufficient clinical height for crown lengthening.

Clinical Findings

  • Tooth #17: Non-tender to percussion (NTTP), non-tender to palpation (NTTPp).

  • Mobility and periodontal depths (PD) within normal limits (WNL).

  • Metallic sound on percussion.

  • Suspected ankylosis or very dense bone.

  • Positive bite test after multiple repeated tests.

Treatment Plan

  • Crown lengthening and redo crown.

  • Extraction (XAP) with possible operation (kiv OP) and replacement with implant or no replacement.

Pre-operative Assessment

  • Reviewed CBCT.

  • Decision made with patient to remove tooth #17 with potential replacement later.

  • Informed consent obtained for extraction of tooth #17.

  • Advised risks of damaging crown on tooth #16.

  • Pre-operative OPG taken.

Anesthesia

Local anesthesia for extraction of tooth #17.

Surgical Procedure

  1. Local anesthesia administered.

  2. Patient cleaned and draped.

  3. Incision made and buccal flap raised.

  4. Bone guttering performed.

  5. Tooth #17 sectioned.

  6. Crown dislodged during elevation.

  7. Coronal portion fractured from heavy elevation.

  8. Minimal luxation of tooth #17 noted.

  9. Tooth elevated out with slight bone from buccal and palatal coming off with tooth pieces.

  10. Curettage performed and irrigated with saline.

  11. BSS x 04 placed for primary closure.

  12. Hemostasis achieved and post-operative instructions provided.
