Chief Complaint
Patient complains of pain in tooth #12.
Clinical Findings
Teeth #11 and #15 sensitive to percussion (STTP).
Tooth #12 very sensitive to percussion (very TTP).
Mobility and periodontal depth (PD) within normal limits (WNL).
Retained carious roots in teeth #11, #12, and #15; very firm.
Treatment Plan
Removal of retained roots of teeth #11, #12, and #15 under local anesthesia (LA).
Pre-operative Assessment
Pre-operative CBCT taken.
Risks and procedures explained to the patient.
Patient understood and agreed to proceed.
Informed consent obtained.
Anesthesia
Local anesthesia infiltration for teeth #11, #12, and #15.
Surgical Procedure
Patient cleaned and draped.
Incision made, and buccal flap raised.
Bone guttering performed.
Tooth sectioning done.
Retained roots of teeth #11, #12, and #15 elevated out in three pieces.
Curettage performed and irrigated with saline.
Bone substitute sponge (BSS) x 06 placed for primary closure.
Hemostasis achieved and post-operative instructions given.
