Chief Complaint

Patient complains of a loose #11 PFM crown.

Clinical Findings

  • #11 crown and cast-post core dislodged fully.

  • #11 retained root with inadequate ferrule.

  • Hopeless retained root.

  • Buccal portion of #11 retained root fractured off and attached to gingiva.

Treatment Plan

Removal of #11 retained root under local anesthesia (LA).

Pre-operative Assessment

  • Risks and procedures explained to the patient.

  • Patient agrees to the procedure.

  • Informed consent obtained for the LA removal of the #11 retained root.

  • Pre-operative CBCT taken, showing #11 with a large periapical (PA) lesion.

  • Mesial-distal crack noted on CBCT.

  • Patient informed of findings.

Anesthesia

Local anesthesia infiltration at #11.

Surgical Procedure

  • Patient cleaned and draped.

  • Incision made, and flap raised.

  • Bone guttering performed.

  • #11 retained root elevated out in two pieces.

  • Curettage performed; radicular cyst and granulation tissue removed.

  • Irrigation with saline.

  • BSS x 03 placed for primary closure.

  • Hemostasis achieved.
