Chief Complaint

Patient reports pain in the third quadrant and a broken tooth.

Clinical Findings

Tooth #34: Caries (distal occlusal) to pulp, subgingival caries, gingival overgrowth into cavity, gross caries under enamel on mesial remnant coronal portion, egg shell hollow core tooth.

Treatment Plan

Extraction (XAP):

  • Possible oral surgery (OP) if complications arise.

Patient Decision:

  • Understood and agreed to proceed with extraction.

Pre-operative Assessment

Extraction Attempt:

  • Local anesthesia extraction (LA XAP) of #34.

  • #34 fractured coronally, resulting in retained root.

  • Elevation and luxation attempted, but tooth was very firm.

Revised Plan:

  • Local anesthesia oral surgery (LA OP) for removal of #34 retained root.

  • Quoted: $800 - $1000 per tooth.

Risks and procedures explained.

Pre-operative CBCT taken.

Patient shown #34 retained root and informed about its multi-rooted nature and proximity to IDN.

Patient understood and agreed to proceed.

Informed consent taken.

Anesthesia

Local anesthesia block (left IDN, LN, and LBN).

Surgical Procedure

  1. Preparation:

  • Patient cleaned and draped.

  • Incision made, buccal flap raised.

2. Extraction:

  • Bone guttering performed.

  • Tooth sectioned.

  • #34 elevated out in 3 pieces.

  • Curettage and irrigation with saline.

  • BSS x 05 placed for primary closure.

  • Haemostasis achieved, post-operative instructions (POI) given.
