Chief Complaint
The patient reports pain and swelling in the area of tooth #27.
Clinical Findings
Tooth #27 has retained roots with caries extending into the furcation and sub-crestal areas.
The tooth is non-vital with pulpitis.
A supernumerary tooth is present on the disto-buccal aspect of #27.
Treatment Plan
Surgical removal of the retained roots of tooth #27 and extraction of the supernumerary tooth located buccally to #27.
Pre-operative Assessment
A pre-operative CBCT scan was taken to assess the area.
The patient was informed about the risks, including the possibility of oroantral or oronasal fistula (OAF/OAP).
The procedures were explained, and the patient understood and agreed to proceed with the treatment plan.
Informed consent was obtained.
Anesthesia
Local anesthesia (LA) infiltration was administered for tooth #27.
Surgical Procedure
The patient was cleaned and draped.
An incision was made, and a buccal flap was raised.
The supernumerary tooth buccal to #27 was extracted.
Bone guttering was performed, followed by tooth sectioning.
The retained roots of tooth #27 were elevated out in two pieces.
Curettage and irrigation with saline were carried out.
Secondary closure was achieved using BSS x 03, ensuring hemostasis and providing post-operative instructions (POI).
