Chief Complaint

Patient complains of pain in the lower right posterior region.

Clinical Findings

  • Tooth #48 impacted with #47

  • Tooth #47 supra-erupted into carious (distal-occlusal) cavity

Treatment Plan

  • Discussion with patient on root canal treatment (RCT) for #47 and removal of #48

  • Patient opted for the removal of both teeth #47 and #48

  • Pre-operative CBCT taken

Pre-operative Assessment

  • Informed of risks including paresthesia to the lower lip and tongue

  • Procedures explained, and informed consent obtained

Anesthesia

Local anesthesia block: right inferior dental nerve (IDN), lingual nerve (LN), and long buccal nerve (LBN)

Surgical Procedure

  • Patient cleaned and draped

  • Incision made, buccal flap raised

  • Bone guttering performed

  • Tooth #48 sectioned and elevated out in two pieces

  • Curettage and irrigation with saline

  • Local anesthesia extraction of tooth #47

  • Irrigation with saline

  • Bioactive synthetic scaffold (BSS) placed x5 for primary closure

  • Hemostasis achieved, post-operative instructions given
