Chief Complaint

Patient reported pain and swelling from fractured, carious retained roots of tooth #27.

Clinical Findings

  • Tooth #27: Fractured, retained roots with large periapical (PA) lesion.

  • Tooth #48: Vertically impacted, keep in view (KIV) with cystic lesion.

Treatment Plan

Surgical removal of retained roots of tooth #27 under local anesthesia (LA).

Pre-operative Assessment

  • Pre-operative CBCT taken.

  • Risks of oro-antral perforation (OAP) and oro-antral fistula (OAF) emphasized.

  • Procedures thoroughly explained.

  • Patient understood and agreed to proceed.

  • Informed consent obtained.

Anesthesia

LA infiltration administered for tooth #27.

Surgical Procedure

  1. Preparation:

  • Patient cleaned and draped.

2. Removal of #27:

  • Incision made and buccal flap raised.

  • Bone guttering performed.

  • Tooth sectioned and elevated out in three pieces.

  • Curettage and irrigation with saline.

  • Five bone support structures (BSS) placed for secondary closure.

  • Haemostasis achieved.

  • Post-operative instructions (POI) given.
