Chief Complaint
Patient reported pain and swelling from fractured, carious retained roots of tooth #27.
Clinical Findings
Tooth #27: Fractured, retained roots with large periapical (PA) lesion.
Tooth #48: Vertically impacted, keep in view (KIV) with cystic lesion.
Treatment Plan
Surgical removal of retained roots of tooth #27 under local anesthesia (LA).
Pre-operative Assessment
Pre-operative CBCT taken.
Risks of oro-antral perforation (OAP) and oro-antral fistula (OAF) emphasized.
Procedures thoroughly explained.
Patient understood and agreed to proceed.
Informed consent obtained.
Anesthesia
LA infiltration administered for tooth #27.
Surgical Procedure
Preparation:
Patient cleaned and draped.
2. Removal of #27:
Incision made and buccal flap raised.
Bone guttering performed.
Tooth sectioned and elevated out in three pieces.
Curettage and irrigation with saline.
Five bone support structures (BSS) placed for secondary closure.
Haemostasis achieved.
Post-operative instructions (POI) given.
