Chief Complaint
Patient presents with retained roots of #22 and #23, both exhibiting significant caries and pulp involvement.
Clinical Findings
Tooth #22: Retained root with gross subgingival caries and caries to the pulp. Very tender to percussion (TTP), with slight metallic sound on percussion. Mobility and probing depth within normal limits. Dense bone and possible ankylosis noted.
Tooth #23: Retained root with gross subgingival caries and caries to the pulp. Impacted by #24, with a large periapical lesion.
Treatment Plan
Surgical removal of retained roots of #22 and #23.
Pre-operative Assessment
Pre-operative CBCT reviewed.
Risks and procedures explained.
Patient agreed to proceed with the treatment plan.
Informed consent obtained.
Anesthesia
Local anesthesia administered.
Surgical Procedure
1. Patient cleaned and draped.
2. Incision made, and buccal flap raised.
3. Bone guttering performed.
4. Tooth sectioning and elevation:
- #22: Retained root elevated in two pieces.
- #23: Retained root elevated in two pieces.
5. Curettage performed and area irrigated with saline.
6. BSS x 07 applied for primary closure.
7. Haemostasis achieved and post-operative instructions provided.
