Removal of Retained Roots at #14 and #15

Surgical Removal

Chief Complaint

Patient presented with pain at the site of retained roots in the upper left posterior region (#14

and #15).

Clinical Findings

#14 and #15 retained roots, grossly carious to the pulp.

Very tender to percussion (TTP) and palpation (TTPp).

Mobility within normal limits.

Probing depths within normal limits.

Slight metallic sound on percussion suggesting possible ankylosis or dense bone.

CBCT revealed #14 and #15 retained roots with gross caries to the pulp and sound

surrounding bone level.

No maxillary sinus or adjacent tooth involvement.

Treatment Plan

Local anesthesia and surgical removal of retained roots #14 and #15.

Curettage and irrigation.

Placement of BSS sutures for secondary closure.

Post-operative CBCT to confirm complete removal.

Post-operative medication and review.

Pre-operative Assessment

CBCT taken and reviewed.

Risks, including possible paresthesia to lower lip and tongue, explained.

Procedure details discussed; patient understood and consented.

Informed consent obtained.

Anesthesia

Local anesthesia administered.

Surgical Procedure

  1. Patient cleaned and draped.

  2. Buccal incision made; mucoperiosteal flap raised.

  3. Bone guttering performed to expose roots.

  4. Tooth sectioning done.

  5. Retained roots #14 and #15 elevated in two pieces.

  6. Curettage and irrigation with saline.

  7. BSS × 06 placed for secondary closure.

  8. Hemostasis achieved.

  9. Post-operative instructions given.

  10. Post-operative CBCT confirmed no remnants.

Post-operative Assessment

Healing satisfactory.

No evidence of root remnants or sinus involvement.

Patient advised rest and medication compliance.

Medical Certificate issued for 5 days.
