Chief Complaint
Patient reported pain and swelling.
Clinical Findings
Tooth #38: Completely buried under gingival, impacted.
Tooth #48: Completely buried under gingival, impacted.
Treatment Plan
Surgical removal of impacted teeth #38 and #48 under local anesthesia (LA).
Pre-operative Assessment
Pre-operative CBCT taken.
Risks of paresthesia to lower lip and tongue, oro-antral perforation (OAP), and oro-antral fistula (OAF) explained to the patient.
Procedures thoroughly explained.
Patient understood and agreed to proceed.
Informed consent obtained.
Anesthesia
Left and right IDN, LN, and LBN blocks administered.
Surgical Procedure
Preparation:
Patient cleaned and draped.
2. Removal of #38 and #48:
Incisions made and buccal flaps raised.
Bone guttering performed.
Teeth sectioned and elevated out completely.
Curettage and irrigation with saline.
Bone support structures (BSS) placed: 4 on #48 and 3 on #38 for primary closure.
Haemostasis achieved.
Post-operative instructions (POI) given.
