Chief Complaint
Patient reported pain and swelling in the lower left jaw.
Clinical Findings
Tooth #38:
Deeply impacted with pericoronitis and pus exudate.
Only 1/8 of the crown visible intra-orally.
2. Tooth #37:
Gross caries (distal, occlusal, buccal) to the pulp.
70% to 80% distal bone loss.
3. Tooth #28:
Non-functional.
Treatment Plan
Extraction of teeth #28 and #37.
LA OP removal of impacted and partially erupted tooth #38.
Pre-operative Assessment
Pre-operative CBCT taken.
Informed patient about the risks of paraesthesia to the lower lip and tongue, and oro-antral perforation (OAP) or fistula (OAF).
Procedures explained thoroughly.
Patient understood and agreed to proceed.
Informed consent obtained.
Anesthesia
Left IDN, LN, and LBN blocks administered.
Local infiltration for #28.
Surgical Procedure
Preparation:
Patient cleaned and draped.
Buccal flap raised with an incision.
Removal of Tooth #38:
Bone guttering performed.
Tooth sectioned and elevated out in seven pieces.
Curettage performed.
Irrigation with saline.
Ten balanced salt solutions (BSS) placed for primary closure.
Achieved haemostasis.
Extraction of Tooth #37:
Elevated out in one piece.
Extraction of Tooth #28:
Local anesthesia administered.
Tooth extracted.
Irrigation with saline.
Achieved haemostasis.
Post-operative instructions (POI) given.
