Chief Complaint

Patient presented with significant pain and swelling.

Clinical Findings

  • Tooth #14:

    • Gross caries to pulp.

    • Very tender to percussion (TTP).

    • Mobility and probing depth within normal limits (WNL).

    • Slight metallic sound on percussion.

    • Possible dense bone and ankylosis.

    • Retained roots.

  • Tooth #25:

    • Perio-endo involvement.

    • Very TTP.

    • Mobility grade II.

    • Probing depth to apex.

    • Hopeless prognosis.

Treatment Plan

  • Extraction (XAP) of tooth #25.

  • Removal of retained roots of tooth #14 under local anesthesia (LA).

Pre-operative Assessment

  • Pre-operative CBCT taken.

  • Risks of paresthesia to lower lip and tongue explained to the patient.

  • Procedures thoroughly explained.

  • Patient understood and agreed to proceed.

  • Informed consent obtained.

Anesthesia

LA infiltration at #14 and #25.

Surgical Procedure

  1. Preparation:

  • Patient cleaned and draped.

2. Extraction of #25:

  • Pus drained.

  • Socket irrigated with saline.

  • BSS placed for haemostasis.

3. Removal of Retained Roots of #14:

  • Incision made and buccal flap raised.

  • Bone guttering performed.

  • Tooth sectioned and elevated out in two pieces.

  • Curettage and irrigation with saline.

  • BSS placed for closure.

  • Haemostasis achieved.

  • POI given.
