Removal of #38, #28

Surgical Removal

Chief Complaint

The patient presents with pain and swelling from a completely buried and impacted tooth #38. The patient requested removal of the tooth.

Clinical Findings

Tooth #38 is fully impacted in bone. The patient has pain and swelling in the area.

Treatment Plan

  • LA OP removal of tooth #38.

  • LA XAP of tooth #28.

Pre-operative Assessment

  • Pre-operative CBCT was reviewed.

  • Risks of paraesthesia to the lower lip and tongue, as well as OAP/OAF, were discussed.

  • The procedures were explained in detail, and informed consent was obtained.

Anesthesia

  • Local anesthesia block was administered to the right IDN, LN, and LBN.

  • Local anesthesia infiltration was performed for #28.

Surgical Procedure

  1. The patient was cleaned and draped.

  2. An incision was made, and a buccal flap was raised.

  3. Bone guttering and tooth sectioning were carried out.

  4. Tooth #38 was elevated in 14 pieces.

  5. The area was curetted and irrigated with saline.

  6. BSS x 04 was placed for primary closure.

  7. Haemostasis was achieved.

Post-operative Assessment

  • Post-operative CBCT was taken.

  • No tooth remnants were noted. The IDN and maxillary sinus were intact and not involved in the surgery.

  • The patient was reassured and provided with post-operative care instructions.
