Chief Complaint
Patient was referred by a speech therapist for tongue-tie release due to functional limitations.
Clinical Findings
Lower lingual frenum noted to be very short and tight
Functional limitations observed:
Restricted tongue mobility affecting speech
Difficulty with food clearance from the buccal sulcus
Treatment Plan
Frenectomy and tongue-tie release of the lower lingual frenum using a combined surgical and laser-assisted approach

Referral from speech therapist reviewed
Indications, risks, benefits, and procedural steps explained
Patient and patient’s father understood and agreed to proceed
Informed consent obtained for lower lingual frenectomy
Anesthesia
Local anesthesia via infiltration
Surgical Procedure
Patient cleaned and draped
Lower lingual frenum placed under tension
Frenum released using a 15C surgical blade
Alveolar gingival tissue dissected away using sharp dissection to partial thickness
Laser used for final contouring and hemostasis
Post-operative instructions provided

Post-operative Assessment
Tongue mobility improved immediately post-procedure
Hemostasis satisfactory
Patient tolerated procedure well
