Patient requires implant placement for missing teeth at sites #25 and #26.

Treatment Plan
Ridge split at #25 and #26.
Internal sinus lift at #26.
Implant placement at #25 and #26.
Pre-operative Assessment
Risks and procedures explained, including ridge split and internal sinus lift.
Patient understood and agreed to proceed.
Informed consent obtained for implant placement at #25 and #26.
Anesthesia
Local anesthesia infiltration.
Surgical Procedure
1. Patient cleaned and draped.
2. Incision made and buccal flap raised.
3. Ridge split performed with piezo electric cutter.
4. Implant sites prepared and ridge expanded.
5. Internal sinus lift at #26 using osteotomes.
Implant Placement:
#25: 3.6mm x 8.0mm Dentium Superline II implant placed with a 4.5mm (S) healing abutment.
#26: 3.6mm x 8.0mm Dentium Superline II implant placed with a 6.5mm (US) healing abutment.
Bone grafting using Ethoss bone grafting material placed buccally at #25 and #26 and between the implants.
BSS x 10 placed for primary closure.
Hemostasis achieved and post-operative instructions (POI) provided.
Post-operative Assessment
Post-operative CBCT taken.
Implant positions verified as good.
Bone graft appears dense.
