Patient requires implant placement for missing teeth at #12, #13, and #23.

Treatment Plan
Implant placement at #12, #13, and #23.
Pre-operative Assessment
Risks and procedures explained to the patient.
Patient understood and agreed to the treatment plan.
Informed consent for implant placement at sites #12, #13, and #23 obtained.
Anesthesia
Local anesthesia infiltration from #14 to #24.
Surgical Procedure
1. Patient cleaned and draped.
2. Incisions made in the 1st and 2nd quadrants anteriorly.
3. Buccal and palatal flaps raised.
4. Implant sites prepared.
Implant Placement:
#13: 3.6mm x 8.0mm Dentium Superline III implant placed with a 4.0mm (S) healing abutment.
#12: 3.6mm x 7.0mm Dentium Superline III implant placed with a 4.0mm (S) healing abutment
#23: 3.6mm x 7.0mm Dentium Superline III implant placed with a 4.0mm (US) healing abutment.
Additional Procedures:
BSS x 05 placed in the 1st quadrant.
BSS x 06 placed in the 2nd quadrant.
Acrylic denture relieved and fit checked.
Hemostasis achieved and post-operative instructions (POI) provided.
