Failure of existing implant at #46 due to worn abutment fitting surface and recurrent crown loosening.

Previously placed Dentium Superline implant at #46
Abutment fitting surface severely worn from prolonged chewing with a loose crown
Implant no longer serviceable
Replacement implant required
CBCT confirmed adequate bone for new placement and safe distance from IDN

Placement of new implant

Incision made; buccal flap raised
Old implant removed using trephine, abutment removal, and reversal driver
New implant site prepared using implant drills and osteotomes
Dentium Superline III implant (6.0 × 8.0 mm) placed at #46
Healing abutment 5.5 mm (S) attached
BSS × 3 placed for primary closure
Hemostasis achieved; POI given

Post-operative Assessment
Post-op CBCT showed correct implant angulation and depth
IDN intact and uninvolved
Patient satisfied with outcome
