Implant Removal and Replacement at #46

Surgical Procedure

Chief Complaint

Failure of existing implant at #46 due to worn abutment fitting surface and recurrent crown loosening.

Clinical Findings

  • Previously placed Dentium Superline implant at #46

  • Abutment fitting surface severely worn from prolonged chewing with a loose crown

  • Implant no longer serviceable

  • Replacement implant required

  • CBCT confirmed adequate bone for new placement and safe distance from IDN

Treatment Plan

  • Removal of failed #46 implant

  • Preparation of new osteotomy site

  • Placement of new implant

  • Primary closure and post-op CBCT verification

Pre-operative Assessment

  • CBCT reviewed with patient

  • Risks and procedural steps explained

  • Implant position re-planned to optimize stability

  • Patient understood and agreed

  • Informed consent obtained for implant removal and replacement at #46

Anesthesia

Local anesthesia.

Surgical Procedure

  • Patient cleaned and draped

  • Incision made; buccal flap raised

  • Old implant removed using trephine, abutment removal, and reversal driver

  • New implant site prepared using implant drills and osteotomes

  • Dentium Superline III implant (6.0 × 8.0 mm) placed at #46

  • Healing abutment 5.5 mm (S) attached

  • BSS × 3 placed for primary closure

  • Hemostasis achieved; POI given

Post-operative Assessment

  • Post-op CBCT showed correct implant angulation and depth

  • IDN intact and uninvolved

  • Patient satisfied with outcome
