Surgical Procedure

Chief Complaint

Missing upper left premolar (#24) requiring implant-supported replacement.

Clinical Findings

  • Edentulous #24 site

  • CBCT confirmed adequate ridge height with need for internal sinus lift

  • Sufficient bone width for implant placement

  • No sinus pathology detected

Treatment Plan

  • Implant placement at #24

  • Internal sinus lift using osteotomes

  • Autogenous and xenograft bone augmentation

Primary closure and post-op CBCT verification

Pre-operative Assessment

  • CBCT reviewed and implant angulation planned

  • Risks and procedure steps explained, including sinus lift considerations

  • Patient understood and agreed

  • Informed consent obtained for implant placement at #24

Anesthesia

Local anesthesia infiltration at #24 site.

Surgical Procedure

  • Patient cleaned and draped

  • Incision made; buccal flap raised

  • Osteotomy prepared using implant drills and osteotomes

  • Autogenous bone harvested from osteotomy site

  • Internal sinus lift performed with osteotomes

  • Megagen AnyOne Implant (4.5 × 8.5 mm, plasma-blasted) placed at #24

  • Buccal flap released

  • Bone grafting performed using autogenous bone + Dentium Osteon II Collagen (6.0 × 10.0 mm)

  • BSS × 5 placed for primary closure

Hemostasis achieved; POI given

Post-operative Assessment

  • Post-op CBCT confirmed dense graft material

  • Sinus lift intact with no membrane perforation or graft leakage

  • Implant position and angulation ideal

Patient satisfied with outcome
