Surgical Procedure

Chief Complaint

Replacement of missing teeth #22 and #23.

Clinical Findings

  • Missing #22 and #23 identified.

  • Ridge width insufficient for implant placement without augmentation.

  • CBCT reviewed showing favourable bone height and need for ridge splitting to accommodate implants.

  • Adjacent structures, including maxillary sinus, not involved.

Treatment Plan

  1. Ridge split procedure to widen the alveolar ridge.

  2. Implant placement at #22 & #23.

  3. Bone grafting with artificial grafting material for volume maintenance.

  4. Primary closure for optimal healing.

  5. Post-operative CBCT and follow-up review.

Pre-operative Assessment

  • CBCT reviewed.

  • Implant position and angulation planned digitally and clinically.

  • Risks, alternatives, and surgical steps explained to the patient.

  • Patient fully understood and consented to implant placement with ridge split + bone grafting at #22 & #23.

Anesthesia

Local anesthesia infiltration administered.

Surgical Procedure

  • Patient cleaned, draped, and prepped under sterile conditions.

  • Incision made; buccal and palatal flaps raised.

  • Ridge split performed using piezoelectric surgery.

  • Ridge expanded with chisels and spreaders.

  • Implant osteotomies prepared with implant drills and bone spreaders.

  • Dentium Slim Line 3.0 × 10.0 mm implants placed at #22 and #23.

  • Bone grafting completed using Dentium Osteon II collagen bone graft material.

  • Flap released and BSS × 10 placed for primary closure.

  • Hemostasis achieved and post-operative instructions given.

Post-operative Assessment

  • Post-op CBCT taken confirming ideal implant positioning.

  • Maxillary sinus not involved.

  • Stability and alignment of implants good.

  • Patient satisfied with the procedure.

  • Healing reviewed and no immediate complications noted.
