Chief Complaint
Patient requires implant placement for the missing tooth #22 site.
Clinical Findings
Tooth #16: Periodontal-endodontic involvement, hopeless prognosis, indicated for extraction (XAP).
Tooth #45: Failed implant, existing 2-unit bridge with #46 implant, requiring sectioning and removal of the failed #45 implant.
Tooth #22: Inadequate bone and severe buccal bony deficiency, necessitating bone grafting.
Treatment Plan
Pre-operative CBCT taken
Discussed risks and procedures, including bone grafting for the buccal deficiency at #22 site
Patient understood and agreed to proceed
Informed consent obtained for implant placement at the #22 site

Anesthesia
Local anesthesia infiltration
Surgical Procedure
General Preparation:
Patient cleaned and draped
Incision made, papilla spared
Buccal flap raised
Implant sites cleaned of granulation tissue
2. Implant Placement at Tooth #22:
Implant site prepared
Mid-operative CBCT taken to confirm preparation
3.6mm x 10.0mm Dentium Superline implant placed at #22 site
5.5mm (small) healing abutment placed
Bone grafting on buccal side using Ethoss (1cc)
Buccal flap released
Bioactive synthetic scaffold (BSS) placed x11 for primary closure
Hemostasis achieved, post-operative instructions given
