Chief Complaint

Patient requires implant placement for the missing tooth #22 site.

Clinical Findings

  • Tooth #16: Periodontal-endodontic involvement, hopeless prognosis, indicated for extraction (XAP).

  • Tooth #45: Failed implant, existing 2-unit bridge with #46 implant, requiring sectioning and removal of the failed #45 implant.

  • Tooth #22: Inadequate bone and severe buccal bony deficiency, necessitating bone grafting.

Treatment Plan

  • Pre-operative CBCT taken

  • Discussed risks and procedures, including bone grafting for the buccal deficiency at #22 site

  • Patient understood and agreed to proceed

  • Informed consent obtained for implant placement at the #22 site

Pre-operative Assessment

Anesthesia

Local anesthesia infiltration

Surgical Procedure

  1. General Preparation:

  • Patient cleaned and draped

  • Incision made, papilla spared

  • Buccal flap raised

  • Implant sites cleaned of granulation tissue

2. Implant Placement at Tooth #22:

  • Implant site prepared

  • Mid-operative CBCT taken to confirm preparation

  • 3.6mm x 10.0mm Dentium Superline implant placed at #22 site

  • 5.5mm (small) healing abutment placed

  • Bone grafting on buccal side using Ethoss (1cc)

  • Buccal flap released

  • Bioactive synthetic scaffold (BSS) placed x11 for primary closure

  • Hemostasis achieved, post-operative instructions given
